About the Role
We are seeking a compassionate and patient-focused Part-Time Nurse Case Manager to support patients, healthcare providers, and clinic partners through a specialized Patient Support Program (PSP).
As a key member of the care team, you will help patients navigate their treatment journey by providing education, care coordination, and ongoing support. You will collaborate with physicians, clinics, pharmacies, and internal stakeholders to facilitate timely access to treatment and help deliver an exceptional patient experience.
This is a part-time position requiring approximately 2.5 days per week. Scheduled hours must fall within the program's operating hours of Monday through Friday, 8:00 a.m. to 8:00 p.m. EST.
What You'll Do Serve as a primary point of contact for patients enrolled in the Patient Support Program.Provide education regarding prescribed therapies, treatment expectations, medication administration, and adherence strategies.
